2.Blood on the tracks

血の轍 - CHI NO WADACHI

Mundane life turns into slow-paced dread and terror in this psychological thriller manga. The premise is simple: Seiichi Osabe lives with his mother, who is often dubbed as overprotective. Her overprotectiveness is not that simple, as it is actually tinged with mental illness, manipulation, and moral decay. No ghosts or demons mark the pages of this work; instead, intricate facial details depicting contorted expressions emanate feelings of morbidity and eerie insidiousness.

3.Dorohedoro

ドロヘドロ

Two worlds co-exist in split dimensions, that of the sorcerers, inhabited by beings who wield magic, and that of the Hole, where lower races dwell and are often subject to the atrocities of young sorcerers' experimentations. As often happens when a ruling group overpowers another, murder, mutilation, and other violent acts are common occurrences between these classes. While the setting of Dorohedoro is quite gruesome, the execution of this manga manages to blend in with comedy and playfulness. Cold-blooded protagonists decapitate one another and go on killing sprees where ample amounts of blood and body parts are lost while managing to crack jokes and eat gyozas on the side.

8.Freesia

フリージア

Freesia takes place in a dystopian society where a law legalizing retaliatory killings has been passed. Japanese citizens now have the right to kill or hire someone to kill murderers of all kinds. As you might grasp from this initial storyline, this isn't a rosy manga. The main protagonist, as well as the set of people we encounter, are almost all devoid of any real saneness. Characters are insane, psychopathic, sociopathic, and sometimes even idealists, becoming licensed retaliatory killers to satisfy a self-righteous need or the growing void they find in themselves. You won't find cocoons of warmth and safety throughout these panels. There is no embellishment or some higher moral grounds. Here, discomfort, gore, and violence rule in an inhumane and disgusting atmosphere.

12.Battle Royale

バトル・ロワイアル

Battle Royal birthed the concept of the Hunger Games. In this alternate world, a class of high school students is chosen every year to participate in ''The Program'', a death game in a deserted area where they are forced to slaughter each other until the last one standing. If they choose to refrain from participating, the collar fitted on their necks will detonate. Battle Royal is not for the faint of heart, it probably contains more violence and gore than a lot of mangas out there, and you'll find body parts mostly residing outside of the bodies they belong to rather than inside of them. But there is more to this work than just gruesome murder and decapitations as it manages to envelop a variety of themes: faith, psychosis, deception, love, hatred, debilitating anxiety, and the feral-like instinct for survival.

One day, he encounters William, an orphan whose intellectual capacities seem to surpass even the most scholar of noblemen. 22.Kami no Kodomo

神の子供 - GOD'S CHILD

Sensitive souls, begone! For this is one of the most monstrous tales I've ever read. Verging on Tim Burton-like bizarre and grotesque illustrations, this psychologically intense manga is not easily digestible - I have yet to put down my hand, shielding my mouth in disgust. It explores a plethora of themes, some of which are so straightforwardly violent and uncomfortable that I hesitated to include it in this list. 'God's child' won't' be the gruesomest read of your life, but it will definitely leave you uncomfortable. Be warned, however, that if you don't appreciate poetic-like narrating and heavy use of nightmarish unrealistic characters and scenes, you probably won't like this one.
